The 5 Beliefs That Block Genuine AI Governance
I wrote a book called Infailible. The title was chosen deliberately. It was a play on words. It was a message that when you place AI at the center of your belief system, you are most likely to fail.
Scroll through LinkedIn on any given morning, and it’s easy to see the ideology take place. Here’s your five-step governance framework. Our product fixes the problem AI created. Autonomous agents, fully capable, ready to deploy. AI this and that!
The belief that AI is simple, reliable, autonomous, and universally capable has become the default narrative. It is also the source of most governance failures. Not because the technology is bad. Even in my book, I stated that it’s not the technology that is the issue, although some may argue that things like hallucinations are part of the reason we need AI governance in the first place.
It’s mainly because organizations build governance on assumptions that are wrong; those assumptions produce governance that addresses imaginary problems while leaving real ones unaddressed.
Here are the five beliefs that do the most damage.
Belief One: LLMs and Generative AI Are the Only AI
The current conversation about AI governance is almost entirely about large language models and generative AI. This is understandable. LLMs are the dominant deployment pattern right now. They are what most organizations mean when they say they are deploying AI.
It’s also the core reason everyone is talking about AI governance. If it weren’t for LLMs, we probably wouldn’t be having this conversation. Every regulatory framework out there is targeting LLMs. Not AI broadly.
The reality is, LLMs are not the only AI. They are not even close to being the only AI operating in most enterprise environments.
Recommendation engines, predictive models, fraud detection systems, computer vision applications, natural language classifiers, anomaly detection algorithms, and robotics systems have been around for 20 years. Organizations have been deploying these systems without applying any of the new AI governance frameworks.
The more dangerous version of this belief isn’t just about governance coverage. It is about tool selection.
When LLMs become the default mental model for all AI, organizations stop asking whether an LLM is the right tool for a specific problem. They start asking how to make an LLM do the thing they need done.
The fear of missing out on what the marketing is hyping leads to bad decisions. Maybe this is less a belief than a desire.
A financial analysis task that would be better served by an analytical model designed specifically for structured data gets handed to a general-purpose language model because that’s what everyone is talking about, that’s what leadership has heard about, that’s what the team knows how to prompt.
I must use ChatGPT because that is what everyone is using.
The governance flaw this creates is the square peg, round hole problem. Organizations end up trying to govern a probabilistic, generative language model, doing a job it was not designed for, wrapping governance around the wrong architecture from the start. Governing a purpose-built analytical model for financial analysis is a different, and more tractable, problem than governing an LLM approximating financial analysis. The governance burden compounds with the architectural mismatch.
Belief Two: AI Is Infallible. Always Right.
It starts from a reasonable observation. AI systems, particularly LLMs, are impressively capable. They produce confident, well-structured outputs. They seem to know a lot. The first ten answers are good. The first hundred transactions process correctly. The first thousand customer interactions produce no complaints.
And then the belief calcifies. The system keeps working. The investment required to verify each instance decreases. Reviews get shorter. Approval rates increase. Edge cases stop generating escalations because the reviewer’s prior is that the system handles them correctly.
The governance flaw this creates is the systematic dismantling of quality checks, guardrails, and responsible AI practices. Not through a deliberate decision to remove them. Through the slow, invisible process of treating them as unnecessary because the system appears to be working.
Organizations that believe AI is infallible stop investing in the mechanisms that catch errors before they compound. Output validation becomes a formality. Guardrails installed at deployment are quietly removed when they produce false positives, slowing things down. Responsible AI reviews that were scheduled quarterly are now pushed to once a year, and then to whenever something breaks. Each of these decisions feels reasonable in the context of a system that keeps performing. Each one removes a layer of protection that existed precisely for the moment when the system stops performing correctly.
The belief in infallibility doesn’t just make governance less rigorous; it undermines it. It makes governance feel unnecessary. And a governance function that feels unnecessary is one step away from being defunded, deprioritized, or quietly abandoned, while the AI system keeps running and undetected errors keep accumulating.
Belief Three: AI Is Autonomous
I have written about this at length. No AI system in production today is autonomous.
And this is a huge problem.
Every system running in any enterprise environment was built by a human, deployed by a human, authorized by a human, given goals by a human, and can be stopped by a human. The governance law comes from outside the system. That is what heteronomy means.
The governance flaw this creates is not just conceptual. It is operational. The belief that AI systems are autonomous produces a specific and deliberate outcome: the offloading of responsibility and accountability.
When the system is framed as autonomous, the human who built and deployed it becomes an observer rather than an actor. “The agent made that decision.” “The model produced that output.” “We didn’t tell it to do that.” The autonomy framing creates linguistic and psychological distance between the human decision-maker and the system’s consequences. That distance is where accountability goes to disappear.
This directly corrupts the human-in-the-loop function. If the system is autonomous, human review becomes a courtesy rather than a governance requirement. The human in the loop is present because it seems responsible to have them there, not because the organization has genuinely committed to the principle that every consequential decision has a human owner. The review becomes ceremonial. The approval becomes reflexive. The accountability chain, which should trace every significant action back to a human decision, terminates at “the system did it.”
The autonomy belief is how organizations offload responsibility without admitting it. Genuine governance requires the opposite posture. Every action traces to a human. Every human is accountable. The system is the instrument. The governance question is always about the people who built it, deployed it, and authorized it to act.
Belief Four: AI Is Easy

The democratization of AI tools is real and valuable. Building AI-powered systems requires less technical expertise than it did five years ago. That is genuinely good.
It has also produced a specific governance failure mode that I don’t see discussed enough.
When AI is easy to deploy, the problems it creates become hidden. The team that built the agent in an afternoon doesn’t fully understand what they built. The integration they connected it to wasn’t reviewed by security. The data it has access to wasn’t scoped by compliance. The policies it should be operating under were never surfaced to the people who wrote them. Everything appears to be working, which means nothing is wrong, which means nobody is looking.
The ease belief creates misalignment in three directions simultaneously. The agent is misunderstood by the people who deployed it. It is misaligned with existing organizational policies that were never consulted. And governance is taken for granted rather than actively maintained, because the deployment felt simple, and simple deployments feel like they should govern themselves.
The governance flaw here is that ease of creation produces an illusion of simplicity that extends to governance. If it took five minutes to deploy, governance should be proportionally simple. That logic produces governance theater. Checkbox exercises that satisfy the feeling of having addressed the problem without building the infrastructure that would make oversight real.
The hardest governance problems are not the ones that look hard. They are the ones that look easy until they aren’t.
Belief Five: AI Can Do It All
This belief is where the other four converge and amplify each other.
Throw AI at the problem. Let’s use AI to do it. It will just work.
The current enterprise AI narrative positions AI as the answer to nearly every operational problem. Automate this. Replace that. Augment everything. And when the system is LLM-based, probabilistic, assumed to be autonomous, believed to be infallible, and easy to deploy, the natural conclusion is that it can handle anything you point it at with minimal oversight required.
The governance flaw this creates is the systematic lowering of guardrails in proportion to the system’s confidence. Each expanded use case brings a new round of “it worked before, it will work here.” Each incremental scope expansion happens without a corresponding governance review. The agent that started with a narrow, well-defined task is now making consequential decisions across domains it was never designed for, under a governance infrastructure intended for the original narrow deployment.
The “do it all” belief loops through every other belief on this list. It assumes that the LLM is the right tool for every job. It assumes the system won’t fail in ways that matter. It assumes that, because the system appears capable, human oversight can be reduced proportionally. It assumes that because deployment was easy, expanding the scope requires no additional governance work.
And when something goes wrong, the response is often to loop back to the beginning. The system failed? Find a better AI. Add another product. The belief that AI can solve the problem it created with AI is the most recursive governance failure of all.
The guardrails that get lowered to deploy AI more broadly are the exact guardrails that would catch the failures that broad deployment produces. Governance that accommodates the “AI can do it all” belief is designed to get out of the way of AI. That is not governance. That is an obstacle course with the obstacles removed.
What These Beliefs Have in Common
Every one of these beliefs reduces the perceived need for governance. LLMs are the only AI, so other systems don’t need governance attention. AI is infallible, so continuous oversight is unnecessary. AI is autonomous, so human accountability is diminished. AI is easy, so governance should be too. AI can do it all, so there are no limits that require governance to enforce.
Collectively, these beliefs produce organizations that believe they are governed while operating systems that are not. The governance framework satisfies the belief rather than addressing the reality. And when something goes wrong, the gap between belief and reality is precisely where the failure lies.
The governance conversation the industry needs is not about which product to buy or which framework to adopt. It is about examining the beliefs underlying governance decisions and asking whether those beliefs accurately describe the technology being deployed.
Most of the time, they are not. And the distance between what we believe AI is and what AI actually is determines whether the governance we build is real or just a more sophisticated version of wishful thinking.
